• Wednesday, March 25: 7:00 AM – 5:00 PM
• Thursday, March 26: 7:00 AM – 5:00 PM
• Friday, March 27: 7:00 – 11:30 AM

This form is to be completed and submitted to the TLA office immediately following any meeting(s) of a Committee, Division, Round Table, District or Discussion Group, with a complete set of minutes to be emailed or mailed to the TLA office within a reasonable time frame.
